Last month, KPN Health attended their second National Association of ACOS (NAACOS) conference, as a NAACOS Business Partner. At the conference, attendees heard critical insights from leading ACO experts covering numerous value-based payment models and trends. KPN is eager to share three top improvement areas covered in depth at the conference:
- Provider Engagement
To better implement successful value-based care, ACOs must strengthen provider engagement. Organizations are discovering that Physician engagement is crucial to a prosperous transition to value-based care models. Coordination improves quality and reduces costs. Higher coordination results in lower cost, while lower coordination results in a higher cost.
KPN Health can help with Provider Engagement providing tools to providers that deliver actionable information to encounters along with open gaps in care utilizing a 360-degree view of the patient.
- Importance of Data Aggregation
At the NAACOS Conference, ACOs highlighted risk management as a top priority. So, it comes as no surprise that organizations are looking for substantial visibility into patient health data through investments in health IT and data management processes. Many Organizations are discovering that their largest problem is that internal IT staff is overwhelmed and in need of assistance. Most organizations do not have enough data analysts on staff to unwrap their network facilities and process external data. This very reason is why many ACOs are going to third-party vendors, such as KPN Health, for support.
“For the past five years we have partnered with KPN Health both directly and through our Accountable Care Organization. During that time, KPN has been very successful in building and maintaining a consolidated community medical database comprised of data from many disparate EHRs in our community.”
James Weissman, MD, Eagle Family Practice
- RAF (HCC)
Hierarchical Condition Category (HCC) is a risk adjustment model used to predict costs for Medicare beneficiaries based on disease and demographic risk factors. The HCC and the Risk Adjustment Factor (RAF) have a significant impact on reimbursement. ACOs experience under-depiction of patient acuity when many patients are reported with RAF scores less than 1. Successful ACOs have developed a disciplined approach and willingness to collaborate with providers, quality, IT and HIM (health information management).
KPN Health can help with RAF by identifying HCC-related ICD codes that have not been addressed in the current reporting year. Providers can then capture any missed-coding at the visit. Additionally, KPN Health helps providers identify persistent codes that can be sent to providers to help capture missing ICD codes. Patient RAF scores are also on Quality reports so providers and quality staff can access and easily create patient projects as needed.
Provider engagement, data aggregation, and risk adjustment factors are just the start to a profitable, value-based payment model. KPN Health’s Point of Care tools and Quality Dashboard are helping organizations, nation-wide, unlock the key to ACO success.
