Hierarchical Condition Category (HCC) is a risk adjustment model used to predict costs for Medicare beneficiaries based on disease and demographic risk factors. The HCC and the Risk Adjustment Factor (RAF) have a significant impact on reimbursement. ACOs experience under-depiction of patient acuity when many patients are reported with RAF scores less than 1. Successful ACOs have developed a disciplined approach and willingness to collaborate with providers, quality, IT and HIM (health information management).
